The Licensed Clinical Social Worker (LCSW) at Providence PACE provides comprehensive biopsychosocial assessments, diagnoses, treatment, and advocacy for elderly patients to support their independence in the community. The role requires a Master's degree in Social Work, an Oregon Clinical Social Worker License, and involves driving to conduct home visits and clinic work. The position emphasizes supporting vulnerable older adults through individualized care within a multidisciplinary healthcare environment.

Description

The Licensed Clinical Social Worker (LCSW) independently provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems. The LCSW incorporates concepts and theories associated with life span development, neurobiology, and behavioral change.

Required Qualifications:

  • Master's Degree - Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE).
  • Oregon Clinical Social Worker License upon hire.
  • Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
  • 2 years - Relevant Social Work experience. May include CSWA experience.

Preferred Qualifications:

  • 1 year - All EMPL's: Care Management: Experience in acute care settings.
  • Social work experience.
