A Shared Living Provider (SLP) , formerly known as Extended Family Homes (EFH) in Nebraska and sometimes called a “Host Home,” is a community-based living option . A contractor with CSN supports a person who lives in their home, which becomes the individual's residence. Qualifications Care experience for individuals with complex physical/medical needs (e.g., hearing impairment, vision impairment, non-ambulatory, etc.) Potential management of behavioral issues such as aggression or property destruction Must be 22 years or older Valid driver’s license and registration Active home/renters and car insurance Reliable computer and internet access Completion of all required training Spare bedroom(s) for individuals to reside Ability to pass State/Federal and APS/CPS background checks Disclosure of all persons residing in the home (background screens required for residences over 13 years old) Certified Nursing Assistant (CNA) certification Responsibilities Providing support ranging from minimal assistance (budgeting, medication management, transportation, activities of daily living) to full support (dressing, toileting, feeding, behavioral support) Assisting with tasks such as cooking, cleaning, laundry, decision-making, and budgeting Skill acquisition and development Medication administration Attending medical appointments and supporting overall health and well-being Daily documentation via web-based case management system (THERAP) Facilitating community inclusion, socialization, and leisure activities Ensuring compliance with all policies and procedures Effective communication with team members Supporting physical and medical needs, including protocols for seizures, falls, feeding tubes, etc. Additional benefits include: Lucrative compensation ranging from $19,000 to $102,000 annually No prior experience required Free training provided 24/7 support from clinical and medical professionals Work as an independent subcontractor Flexible schedule and time off based on individual needs Routine respite care available for vacations or personal time We value each client and their families, ensuring respect and dignity for everyone. #J-18808-Ljbffr Remedy Road LLC.
