Remote RN Case Manager - 13-Week Contract | $42/Hour (Paid Weekly)

Location: Remote (Must be licensed in Michigan)
Schedule: Full-Time (40 hours/week) | Day Shift
Pay: $42/hour | Weekly pay
Contract Length: 13 weeks
About the Role:

Our hospital client is seeking a compassionate and experienced RN Case Manager to join our team on a contract basis . In this remote role, you will coordinate patient care, manage case workflows, and ensure optimal health outcomes-all while working from home. Key Responsibilities:
• Oversee and coordinate patient care plans in collaboration with healthcare providers
• Conduct virtual assessments, monitor progress, and ensure continuity of care
• Provide education and support to patients and families regarding treatment plans
• Utilize clinical expertise to manage transitions of care and discharge planning
• Maintain accurate documentation within electronic health records (EHR)
• Work efficiently within a remote team setting while adhering to compliance standards
Requirements:

Active RN license in Michigan (required)
Minimum 2 years of case management experience (preferred)
Strong communication and decision-making skills
Familiarity with EHR systems and remote patient coordination
Ability to work independently and meet performance metrics

Bachelor's degree in nursing or related professional field (i.e., social work, counseling, health education, etc.) or a Master's degree of Social Work, required.
Minimum (3) three years of clinical experience, required.
Registered Nurse (RN) or a Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license, required.
Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association.
