As a CNA, you’ll provide essential support and personal care to patients in their homes, helping them maintain comfort, dignity, and quality of life during their end-of-life journey. You’ll work under the supervision of a Registered Nurse and collaborate with a team that values empathy, respect, and dedication. Responsibilities Provide direct patient care according to the Aide Plan of Care and under RN supervision Assist with daily living activities including hygiene, grooming, dressing, and mobility Help patients with safe transfers and ambulation Monitor food and fluid intake Maintain a clean, safe, and respectful environment Document and report patient conditions and changes to the care team Support infection control and safety standards Uphold hospice philosophy and promote a respectful, compassionate atmosphere Qualifications High school diploma or GED (preferred) Certified Nursing Assistant (CNA) certification in good standing CPR certification (required) At least one year of experience preferred in hospice, home health, or long-term care Reliable transportation, valid driver’s license, and auto liability insurance Ability to lift up to 50 lbs and assist with patient mobility Excellent communication, time management, and interpersonal skills Ability to work independently and adapt to various home settings Spanish speaking is a plus Why Harmony Hospice Care?
