The Care Manager LCSW is responsible for coordinating patient care through collaborative communication with healthcare professionals, addressing psychosocial needs, and managing resources across medical and community settings. This role requires a Master's in Social Work, a clinical social worker license, and experience in care management and behavioral health. The position supports patient-focused, whole-person care in a hybrid work environment within a large healthcare organization.

Care Management is a collaborative practice model including the patients, nurses, Care Manager LCSWs, physicians, other practitioners, caregivers and the community. The care management process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination and addressing the psych social needs of the patient.
